A University Judicial System complaint can be filed against any student or student organization, or any member of the University community. A student, faculty member, or staff member may file a complaint by completing the standard complaint form, which is available in the Office of Judicial Affairs, or as a PDF at the link below.
When filing a complaint, it is very important that all documentation and other evidence associated with the complaint (e.g., Department of Public Safety reports, police reports, or witness statements) be included with the standard complaint form and returned to the Office of Judicial Affairs. More than one complaint can be filed regarding the same incident. It is absolutely necessary that all complaint forms be signed by the complainant, whether they are obtained in person or downloaded from the link on our website. The complaint form will be considered complete only when it is signed and dated by the complainant.
